What is this brake disc?

It's a Ferodo replacement brake disc, the rotor that the pads clamp onto to slow the wheel. Discs wear down in service and are replaced when they drop below minimum thickness, or when they're scored, cracked or heavily corroded.

Should I change discs and pads together?

Fit them together where you can. New pads bedding onto a worn disc take up the old wear pattern, so braking stays uneven and the pads wear quickly. Discs should also be replaced as an axle pair rather than one side at a time.
