The hose carries vacuum from the engine vacuum pump to the brake servo (booster), providing the reduced pressure the servo uses to multiply pedal effort. A perished, split or collapsed hose loses vacuum and results in a hard brake pedal with reduced assistance, so the hose is replaced to restore full servo operation.
